Preparation for Parents

Providence requires answers to the following questions when a parent calls 255-5577 to enquire about registering their child.

• Name
• Address
• Telephone
• Fax
• Email
• Name of child
• Child's Date of Birth
• Does your child have a disability?
• If yes.
• What is the disability?
• Is your child presently being seen by a therapist or has been seen previously?
• If yes to either:
• Please supply specific information, including the date.
• Do you wish to have full or part time days at the Centre?
