Registration Process and Requirements

Once you have made the decision to register your child for the LEARN Program, an online registration package will be emailed to you. The following documentation must be submitted with your registration form:

• Your child’s birth certificate. If your child was not born in Canada, please provide a copy of Permanent Resident/Landed Immigrant status/Child of a Canadian Citizen.
• Copies of your child’s therapy reports, assessments and/or a doctor’s letter stating your child’s medical diagnosis.

Once you have completed all registration requirements, Providence will apply for Program Unit Funding (PUF) for your child.
